چکیده

Tunneled dialysis catheter tip design may affect freedom from dysfunction and/or infection; improved performance within a critical care setting reduce costs and need for guidewire exchange or removal. This study compared rates of failure between symmetrical, helical-tip (VectorFlow) with conventional split-tip (Ash-Split) among ICU patients undergoing tunneled placement. A retrospective database identified total 1734 catheters placed at an academic institution over 7-year period. Of these, 340 (19.6%) were patients, which included VectorFlow (n = 112, 32.9%) Ash-Split 228, 67.1%) catheters. Rates requiring removal composite outcome malfunction infection Kaplan-Meier estimates, covariates potentially influencing assessed Cox proportional hazards modeling. Catheter occurred 90 days in 34.8 ± 3.4% patients. Among catheters, primary unassisted patency 30, 60, 180 was 87.4 3.6%, 78.0 5.1%, 75.1 5.0% 60.1 8.2% 75.0 3.3%, 62.8 4.1%, 60.7 4.2% 44.3 5.5% (P 0.022). modeling body mass index, patient age, sex, left jugular (vs. right) placement indication (initiation hemodialysis access vs. failed surgical arteriovenous access) showed no effect on patency; had hazard ratio 1.72 times higher than 0.024) univariate 1.81 multivariate 0.017). The associated significantly the independent multiple clinical anatomic factors. These data support VectorFlow-first strategy this population.

Which general intensive care unit patients can benefit from placement of the pulmonary artery catheter?

From the report by Connors and coworkers in 1996 until now, much effort has been directed at demonstrating the safety and/or effectiveness of strategies based on pulmonary artery catheter (PAC) data. Although studies have failed to demonstrate a clear benefit of PAC use, neither have any corroborated the initial report of PAC-induced mortality. With this in mind, it is important to clarify the ...

Peritoneal Dialysis Catheter Placement Techniques

The success of catheter placement is critically important for the ability to stay on peritoneal dialysis over the long-term. Nephrologists have used percutaneous placement with or without fluoroscopic guidance and placement using peritoneoscopic guidance to place these catheters. Complications can be divided into early, within 14 days, and late complications, those arising more that 14 days aft...

Comparison of the Effects of Intralipid 10% and 20% on Peripheral Catheter Ablation in a Pediatric Intensive Care Unit

Background: Energy deficit is a common and serious problem in pediatric intensive care units. Parenteral nutrition, either alone or in combination with enteral nutrition, can improve nutrient delivery in critically ill patients by preventing or correcting the energy deficit and improving the outcomes. Intralipid 10% and 20% are lipid emulsions, widely used in parenteral nutrition. Despite sever...

Interventions to decrease catheter-related bloodstream infections in the ICU: the Keystone Intensive Care Unit Project.

BACKGROUND A quality improvement initiative that included rigorous measurement, feasible interventions, and cultural change was shown to nearly eliminate catheter-related bloodstream infections (CR-BSIs) in patients in a surgical intensive care unit (ICU). To build on this research, a statewide collaborative cohort study was conducted using the same evidence-based interventions. METHODS Inter...
